Resident Biography Instructions

Dear Resident or Family Member:

At the Villages, we enjoy getting to know about the lives of our guests - and we would love to learn more about you or your family member. We use this information within our community for staff education and to highlight our residents' lives.

Our Resident Biography form can be filled out online or download and mailed to us. Please make sure that you also fill out or send back our Release Form, as we will need this as well.

If you would like to submit photographs with the biography, please attach or include them when you send or mail the biography and release forms.

We look forward to getting to know you and/or your family member better!
